If you are filling out this google form, you are informing us of the following:

"My student cannot attend the winter concert due to a prior conflict, out of concern for a student or concern for a high risk family member. I am informing you with enough time in advance to receive the alternate assignment to be completed. I understand that this concert is an important component of our performance-based fine arts class and this performance counts as a final exam GRADE for the class.

I also understand that this alternate assignment my student will complete is due by our band's final exam date."
