This form is to be completed by the PARENT, not the student. If you have multiple students in the touring choirs, please only complete one ballot per household/family.

PLEASE REMEMBER THAT THIS VOTE IS NOT WHETHER OR NOT YOUR STUDENT WILL BE PARTICIPATING IN MUSIC TOUR 2022. IT IS A VOTE OF WHETHER YOU, AS A COMMUNITY MEMBER INVOLVED WITH MVHS MUSIC, SUPPORT THIS TOUR PROPOSAL AS A WORTHWHILE SCHOOL ACTIVITY. An 80% positive response is necessary before submitting our application to the School Board.

Location: Anaheim, CA
Dates: Wednesday, March 16 (after school) - Monday, March 21 (Evening), 2022
Cost: $750
Mode of Transportation: Coach Bus
Hotel: Embassy Suites

Possible Musical Opportunities: high school exchange with local high school (Las Vegas Academy of the Arts); performance at local care home or retirement community

Possible Activities: Disneyland

Fundraising: Starving Student Cards, Krispy Kreme Cards
