Dismissal Change Request Form
Sussex Montessori Charter School requires that parents/guardians submit a written notice for any/all cases of dismissal changes.  Communication must be received by the school office by 12:00pm (noon), on the day the dismissal change is to be effective

* Any dismissal change request received after 12:00pm on the effective day of the change, CANNOT be guaranteed, as the school needs to time to make the necessary arrangements to ensure the safety and security of your and all students.
