填寫借用此表單前：
1.請先詳閱「本校安全衛生工作守則」(學校首頁/行政單位/環境安全衛生中心/相關法規/職業安全衛生管理/本校安全衛生工作守則 https://ppt.cc/fYihEx)。
2.請先完成 實驗(習)場所新進人員安全衛生教育訓練計畫 之系列課程。
3.下載「設備借用同意書」(https://reurl.cc/g0nDn4)，請借用人及所屬單位負責人(如指導教授)親簽後，請將正本繳至 F403給邱詠恬助教 或  F413給夏文昱助教。

請注意：
1. 上述3點完成後才能填寫本表單
2. 僅受理當學期之借用，無法受理跨學期借用。
3. 最晚必須於一天前完成申請。同時有多人使用時，所有使用者亦需登記。
4. 於週間上班時間借用，若有特殊情況或需要其他使用時間請與實驗室負責人聯絡。
5. 進入實驗室記得掃描門口QRcode實名制，全程配戴口罩及護具。
6. 使用完畢務必須關閉儀器電源，關閉水源，清潔環境。

To all users:

Fill out this form before borrowing:
1. Please read the “University's safety and health working rules“ (School homepage / Administration units / Environmental Safety and Health Center / Regulations / occupational health and safety management / The school safety and health working rules https://ppt.cc/fYihEx).
2. Please complete the course of “safety and health education and training plan for new employees in the laboratory” first.
3. Download the "equipment loan agreement”(https://reurl.cc/g0nDn4), please sign it by the borrower and the person in charge of the unit. And hand in the original one to F403 to Yung-Tien or F413 Wen-yu.

NOTE:

1. Fill in this form only after completing the above three points.
2. The signed document is only good for one semester only. Re-application is required for every semester.
3. All bookings must be completed one day ahead of the usage date. All users need to login for booking even other people had booked at the same time.
4. Booking and using the facilities are only allowed during working days and working hours. Please contact the responsible managing professor for a special case.
5. Remember to scan QRcode for conduct name registration when entering the laboratory, and wear a mask ＆ protective gear during the whole process.
6. When you finish using the facilities, must turn off the power, turn off the water supply, and clean the environments.
