This health screen should be filled out by dancers before EVERY in-person private lesson. This checklist applies for all children, as well as all students who attend kindergarten to Grade 12. Children may need a parent or guardian to assist them to complete this screening tool.

If an individual answers YES to any of the questions, please contact the studio to arrange to attend class virtually. If you have a private lesson booked: contact the studio to re-book your lesson. There is no financial penalty to re-book a private lesson.

Does the child have any new onset (or worsening) of the following symptoms: chills, sore throat/painful swallowing, runny nose/congestion, feeling unwell/fatigued, nausea, vomiting and/or diarrhea, unexplained loss of appetite, muscle/joint aches, headache (not related to a known cause), conjunctivitis (pink eye)? *

If you have answered YES to ONE symptom in question 2: Keep your child home and monitor for 24 hours. If their symptom is improving after 24 hours, they can return to dance when they feel well. If you have answered YES to TWO OR MORE symptom in question 2: Keep your child home and complete the AHS Online Health Assessment Tool to determine if testing is recommended. https://myhealth.alberta.ca/Journey/COVID-19/Pages/COVID-Self-Assessment.aspx  You can return to dance once symptoms go away as long as it has been at least 24 hours since symptoms started.
