This questionnaire assesses 5 dimensions within the Big Five personality model with each 25 facets as described in the DSM-V. This version is NOT intended for commercial usage but only for trained professionals like psychologists and/or psychiatrists.
Clients who fill out this PID-5 will automatically get an anonymous report of their answers to this questionnaire. They can email this report to their therapist.
Please realise that filling out this questionnaire will take you approximately 40 to 45 minutes. There is however a possibility to pause and resume at a later date but this option is NOT recommended and must only be used if you really have to interrupt the questionnaire.

This service is provided for free due to the following reasons::
1. It is much more client friendly: you can fill the PID-5 out on your mobile phone, tablet or computer. Scoring of your answers is swift and errorless.
2. A much quicker diagnostics of personality problems will certainly improve and speed up treatments in the mental health profession.
3. As acting clinical neuropsychologist I am interested in collecting data about the prevalence of personality disorders of clients in the mental health profession. Hopefully such data can somehow improve the treatment of personality problems.
4. By filling out the PID-5 you have to AGREE with the temporary storage of your email address. Please read the paragraph about Privacy below very carefully.

PRIVACY:
As a clinical neuropsychologist I am committed to my professional code of my professional association NVGzP. That means that your data are being stored as anonymously as possible and I have a professional secrecy. My registration number in the Netherlands is 49050080925 and you can find me at www.bigregister.nl.
Your data will be processed statistically and completely anonymously. The analyses done have absolutely no means of tracing back to you as a person.

To receive a report of your answers filled out in the PID-5, in a safe way, you have to be logged into your own Google account. Furthermore, you need to enter an email address so that this service knows where to send the report to.
Please note that this email address will be destroyed within 30 days so that you can NOT receive a report anymore! In the unfortunate case that you have not received an automatic report you can send a request to me at 3zpsychologie@gmail.com within a week after filling out the PID-5.

An advice: if you are still uncomfortable in entering your own email address you can also simply make a new dummy email account in Google that does not reveal your name.
