Prior COVID Infection Report Form
If your player has had a previous positive COVID test result and has since recovered, please fill in the information on this form. Having this information on hand will help us expedite notifications to coaches and players/families as needed throughout the season regarding potential COVID exposures.

Per the CDC:
 "If you have recovered from COVID-19 in the past 90 days and have close contact with someone with COVID-19, you do not need to quarantine if ALL of the following are true:

Your illness was confirmed with a positive lab test in the past 90 days.
You have fully recovered.
You do not currently have any symptoms of COVID-19."

MN River Hockey uses the honor system for reporting, please do not send test results. If you have multiple players to enter please fill out an individual form for each player.
