    Current Headshot
    Birth Certificate
    Copy of Health Insurance Card (front/back)
    2022 USL League Two Amateur Registration Form (Fillable
    2022 USL League Two COVID-19 Agreement

INTERNATIONAL BORN PLAYERS will need:

    Current Headshot
    Copy of Passport, Visa, Green Card or Valid Permanent Resident Card
    Copy of Health Insurance Card (Front/back- if you have insurance)
    2022 USL League Two Amateur Registration Form (Fillable)
    USSF_ITC_Request_Form (If you were born outside of the U.S. or have played outside of the U.S., you MUST complete this form)
    2022 USL League Two COVID-19 Agreement
