This form should be filled out no later than June 7, 2019 for tournaments held on June 8, 2019, no later than June 14, 2019 for all tournaments held on June 15, no later than June 21 for tournaments held on June 22, and NO LATER THAN JUNE 24 FOR ALL TOURNAMENTS JUNE 29-JULY 27, 2019.

I understand that teams seeking to qualify eligible players for the national championship via my tournament *must* play on the roster of the club being used in qualifiers, and that all players for the team(s) in my tournament must be affiliated with said club(s) *
Example: If a player is playing for a team called Team Blue in qualifying tournaments, the team must participate as Team Blue at an approved tournament and only use players registered to Team Blue with USA Rugby (public rosters available at: https://www.usarugby.org/membership-resources/public-rosters/)
