SUMMER MEAL PLAN PROGRAM:
TERMS: A PolarExpress card or Housing Access card is required to access meal plans at any UAF Dining Services location and on‐campus vending machines. Requests for meal plans will be accessible on the designated card either on the first day of the activation period or within 24 business hours if requested during the activation period. Lost or stolen PolarExpress cards must be replaced at the PolarExpress office located next to the Office of the Bursar in Signers’ Hall. Lost or stolen Housing Access cards must be reported to Conference Services in Residence Life. Once replaced, updated card information must be given to Dining Services to transfer funds.

These plans are non‐transferable to another card user.

ACTIVATION PERIOD: May 8, 2023 - August 31, 2023

EXPIRATION: 2023 Summer meal plans will expire September 1, 2023.

REFUNDS: No refunds will be granted for the purchase of a summer meal plan.

LOCATIONS AND HOURS OF OPERATION: Information for all Dining locations and hours of operation may be found at www.uaf.edu/dining. NOTE: Food service on campus is limited during summer and university holidays. Adjustment to hours will be posted in advance at all locations and on the above website.

UNIVERSITY AUTHORITY: The University reserves the right to:
a) Close food service facilities for maintenance or an emergency with or without advance notice.
b) Terminate or temporarily cancel individual meal plans privileges upon indication of non‐payment of fees or misuse.

By signing this agreement, student affirms the information provided on this application is true and is in agreement with any additional charges that may be added to their account resulting from the selection on this form. The student agrees to pay any attorneys’ fees and collection costs, which may be based on a percentage at a maximum of 40% of the debt, necessary for the collection of any amounts owed to University of Alaska. If student does not pay, University of Alaska may take student’s Permanent Fund Dividend under Alaska Statues 14.40.251 and 43.23.073 and pursue other collection methods.
