Use this form to sign off data about trainees who studied in the 2021 to 2022 academic year.

You must sign off the data by 31 January 2023 at 11:59pm. Some of the data will be included in the annual initial teacher training (ITT) performance profiles publication.

By signing off your data, you agree that it is correct. The email which contains the link to this form also has instructions for checking your data and fixing any mistakes.

Who can sign off the data

A senior person from your organisation must sign off the data. This should not be the same person who entered the data.

If you are not the right person to sign off the data, you should share this form with someone senior in your organisation.

Which email address to use for this form

You should use a work email address which includes a person’s name, for example jane.doe@organisation.ac.uk or jdoe@organisation.ac.uk.
