This form is only to be used for quarantining, feeling sick, unforeseen circumstances or have spoken with a Psych Club officer over a very strict time conflict (must be approved).

Please have this submitted at least 24 hours before the meeting you are planning on attending virtually!
We understand this may not be applicable for unforeseen circumstances and falling ill.
At worst case scenario, make sure to submit a request form an hour before the meeting time!!

We will send you a zoom link to the email you submitted at least10-15 minutes before the meeting!
