The initial document-gathering stage is the least enjoyable part of the mediation process but necessary to gathering all important information. This checklist is a resource for you to gather any essential documents needed during this process.

Current bank account statements

All children's accounts

Stock and bond investment statements

Copies of paystubs/income statements of each spouse for the previous 6 months

Taxes

Copies of state and federal tax returns for the previous (3) years

Copes of corporate tax returns for the previous (3) years if one of both spouses have a business

Insurance

Declaration sheet for all life insurance policies held by either or both spouses, and a statement of the cash surrender value of any whole or universal life insurance plans.

Declaration sheet for all disability policies Other Related Marital Documents

Copies of any trust documents

Copies of all pre-marital, and/or marital agreements

Name and date of birth of each child you have together (whether minor or of majority age)

Copies of any wills executed during the marriage

Make, model and year of all vehicles owned and a statement of the private party value for each

Pending civil lawsuit claims

All retirement account statements owned by each spouse

Employment benefits

Statements (i.e. stock options, incentive, cash balance or golden parachute plans.)

Market value appraisals for all real estate owned (unless spouses agree that the property is being placed for sale.)

Personal property--approximate value of (i.e. jewelry, art, antiques)

Business appraisals

Loan statements

Liabilities (current balance statements)

Mortgage(s)

Home equity loans or lines of credit

All loans (vehicle, student, private)

Credit card accounts

Pending civil lawsuits

Post-Divorce Financial Checklist

Cancel/suspend joint accounts

Change beneficiaries on all accounts

Update your personal insurance coverage (Auto, homeowners, umbrella liability coverage)

Create a new estate-plan (Living trust)

Retitle assets in your own name or the name of the new trust

Analyze your investments (Risk/reward, income or growth)

Create new financial plan (budgeting, cash flow analysis, emergency reserve account *6 months of covered expenses)

Check safety deposit box

Shred all old credit cards, offers, and statements
