❗ It is important to note that you will need to be able to commit to
weekly one-hour sessions for a period of
6-9 weeks at the agreed time. There are time slots shown below, please give several options to enable us to assign counsellors to your case. Your sessions will be the
same time and day each week, so please ensure you have the capacity to do so before completing this form.
Details of BOTH parties must be filled in. Both parties must be willing to attend counselling sessions otherwise an Individual Counselling Request should be submitted for individual counselling.
