BIRTHDAY SIGN FORM
Celebrate your child's birthday with a shout out at school! 
Birthdays will be posted on screens in the main hall and on the outside marquee. 
  • Virtual Birthday Sign Form-Please complete this form online. If you prefer or need a paper form, you must request one from your child's teacher. 
  • *To guarantee your child's birthday is posted on the correct date, please turn in a form at least 7 days in advance* Entries are not able to be checked daily. However, this form can be turned in throughout the year. 
  • If choosing Design #1 with the picture option & picture isn't received, another design will be chosen instead. 
  • Weekend birthdays will run inside on a Friday and on the outside sign on their actual birthday. Summer birthdays can be celebrated at the end of the year or during their 1/2 birthday. 
  • We must receive payment (cash or check) before we post the birthday announcement. 
  • Please return money to the child's teacher. Cost is $15 with funds going to the CTE PTA.
    (Birthday will be posted inside & outside signs-the outside sign will only list their first name)
