Chino Valley Unified School District is preparing to begin the Hybrid Learning model on September 8th.  In this model, for safety purposes, no more than half of the students will be at school each day in order to maintain social distances in classrooms and on buses.  Families who choose Hybrid Learning will be assigned 2 days per week in the classroom (either Monday/Wednesday OR Tuesday/Thursday) combined with two days of distance learning.  Families and students in the same household will be assigned the same days.  Families choosing Hybrid Learning should know that an executive order from Governor Ducey requires students to wear a face covering while on district campuses and district buses.  Students will have their temperatures taken as they arrive onto campuses. Additionally, a COVID-19 waiver must be signed and returned to the school prior to attendance.

Families who are NOT comfortable sending their student(s) to school may choose to continue At Home Learning (online learning).

With this form you will be registering your student for Hybrid Instruction (a combination of in-person and distance learning) or remaining with At Home Learning.  This decision will be effective September 8th and will continue through at least October 8th.  At Home Learning will continue to remain the only model of instruction until September 3rd.

Please complete an online Hybrid Instruction/At Home Learning registration form for each of your students in grades kindergarten through 12th grade.  You do not need to complete this form if you already turned in a paper Hybrid Learning/At Home Learning registration form for your students.  

Preschool students will receive information separately regarding preschool instruction beginning September 8th.
