Use this form to request a Certificate of Completion. Keep in mind that OPA offers Certificates and Verifications; Certificates take a long time to produce and are for hanging on the wall; Verifications are fast to produce and are used for job applications. You may request both, but a common mistake is postdocs who need a Verification (which can be produced quickly) mistakenly request a Certificate (which takes a long time to produce) instead. You can request a Verification here: https://postdocs.stanford.edu/about/verifications
