Complete this form once for each person that wants to attend EM worship in person at church this week. Note:
1) We are only able to accommodate a reduced capacity of 50 at present (this form will automatically close)
2) A new form will be created each week, with the new link posted on the website under announcement links
3) Priority may be given to others that have not yet been able to join worship in person on previous weeks
