Pizza ORDERS AND PAYMENT for this month are due on 9/9. Pizza DELIVERY for this month will be served on 9/16 and 9/30.    
 1. Students must order pizza slices of the same variety in order to efficiently serve the volume of students we have at NOCA.
2. ONE ORDER FORM PER STUDENT is required.
3. Families may consolidate payments by writing ONE check payable to Northwest Ohio Classical Academy. Cost: $2.50 per slice of pizza.
4. Only electronic orders placed on this form by the deadline will be accepted.
5. Please send payment with your child to school in an envelope with your child's name marked on it.
6. Payments will be submitted to the office by the classroom/homeroom teacher by the designated due date.
