Thanks so much for your interest in being a vendor for our TENTH ANNIVERSARY SHOWCASE!  Participating in a vendor event for Mayne Attraction Studios means you'll have the opportunity to sell your products and/or services to an audience of over 250 people! Please complete this interest form NO LATER THAN SATURDAY 4/8/23, and a member of our team will contact you with more details! 

After completing this form, please pay your $75 vendor fee via cash app to $MayneDance or paypal.me/mayneattraction. Please include your name and business name in the subject line when submitting payment. Our spring showcase is scheduled for Saturday 5/20/23 at Cleveland Heights High School from 10:00am-2:00pm. VENDOR FEES ARE DUE NO LATER THAN SATURDAY 5/6/23.

As a result, I acknowledge and agree that neither Mayne Attraction, nor its owner(s), instructors, and/or staff, shall bear any responsibility and/or liability for injury, illness, etc. that may result from participation in Mayne Attraction classes, performances, or related functions.  *
I understand that participating in vendor events hosted by Mayne Attraction Studios increases the possibility of selling my products/services to a new group of potential clients, but Mayne Attraction Studios DOES NOT GUARANTEE sales as a result of participating in MAS vendor events.  *
I give my permission for photographs, videos, or any media footage that includes myself and/or members of my business/team to be used for promotional purposes on any form of Mayne Attraction advertising. *
I understand that my vendor fee is non-refundable, but can be used toward a future Mayne Attraction vendor event if I need to cancel.  *
