The Ely Senior Center may be reserved by filling out an application and submitting it. Drop off your security deposit at the Ely Senior Center, or mail it to:

Security deposit will be refunded in full providing there is no evidence of damage and the facility is left clean.

I assume responsibility for any physical damage incurred as a result of activity or attendants at my event.

I am responsible for all persons using the facility and hold Ely Senior Citizens Inc. harmless from any claims

I am over the age of 18 and will be present for the duration of the activiity.

If I am using the kitchen, I have a Serve-Safe certificate OR have taken the Ely Senior Center mini class

If alcohol will be served, I will provide the necessary certificate of insurance. Cost varies by type of event and number of attendees from $120 to $235. You can purchase insurance from www.theeventhelper.com#HE9Ee0

There may be other events which are required to obtain liability insurance.

It is my responsibility to leave the facility in good order (see Rules Checklist below).

Rules Checklist

Please checkmark each rule. Your checkmark is acknowledgement that you understand each rule.

Fire regulations prohibit more than 200 people in the building.

Vandalism will not be tolerated. If property is destroyed, the person using the facility will be held responsible and charged for all damages.

Young children should always be chaperoned.

No animals are allowed in the building.

No smoking or use of tobacco products will be allowed on the premises.

No tacks, staples or masking tape are allowed on the walls, floors, doors - minimal scotch tape or blue painters tape may be used.

Do not drag tables and chairs across the floor. Any tables or chairs that are used must be cleaned and put back in their proper place.

It is the responsibility of the applicant to remove and dispose of all garbage including kitchen and bathroom areas. You must provide your own garbage bags.

Floors must be swept, tables wiped. Kitchen left as found.

If used, the kitchen must be cleaned per health regulations.
