SSWSC Event COVID Pre-Screening Questionnaire
Per U.S. Ski & Snowboard requirements, all participants in U.S. Ski & Snowboard events must complete this form within 24-48 hours prior to the event, and will be required to complete a daily symptom check on the morning of each competition day. Participants include athletes, coaches, officials and volunteers.

You understand that you must self-screen for COVID-19 symptoms each day of the competition, including taking your temperature. You will report any symptoms to your coach, team captain, or event COVID coordinator listed on event fact sheet.
