Our New Beginnings program has become EXTREMELY successful these last couple of years and the demand is beginning to outpace our resources. The purpose of this form is to obtain the information necessary to consider providing back to school assistance to those who are truly distressed and facing an extreme financial need. It is our sincere desire to help alleviate the burden of those in the greatest need.

WHAT HAPPENS AFTER I FILL OUT THIS FORM?
A committee of New Beginnings leaders will follow up with each person filling out this form and confirm exactly what is needed.

WHAT IF I CHOOSE NOT TO FILL OUT THIS FORM?
Those that do not fill out this form will not be considered to receive a back to school box from New Beginnings.
