Information clause: Application for participation in the Black Horse of Music Videos competition
Based on Article. 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/96 / EC (Official Journal EU L 2016, No. 119) hereinafter "GDPR", I inform that:
1. The administrator of your personal data is Katowice City of Gardens, Plac Sejmu Śląskiego 2, 40-032 Katowice, hereinafter referred to as the Administrator; The administrator conducts the processing of your personal data.
2. Contact with the data protection officer is possible at:

3. Your personal data will be processed in order to participate in the Black Horse of Music Videos competition and will not be made available to other recipients, unless it results directly from the law.
4. The basis for the processing of your personal data is the performance of a task carried out in the public interest - art. 6 sec. 1 lit. e) GDPR.
5. Scope of processed data: name, surname, e-mail address, number of people/registration.
6. Providing your personal data is voluntary but necessary to achieve the goal of participating in the event; in the case of failure to provide data, it is impossible to implement the above-mentioned purpose.
7. On the basis of applicable regulations, you have the right to:
- access to your personal data (Article 15 of the GDPR),
- rectification of your data (Article 16 of the GDPR),
- deletion (Article 17 of the GDPR) or limitation of the processing of personal data (Article 18 of the GDPR),
- data portability (Article 20 of the GDPR),
- object to such processing (Article 21 of the GDPR),
- lodge a complaint with the supervisory authority (Article 77(1) of the GDPR),
8. Your personal data is not subject to automated decision making, including profiling
(pursuant to Article 22 of the GDPR).
9. Your personal data will be stored until the closing date of the festival. In the case of competition winners - your personal data will be stored for 6 years in accordance with Article 70 § 1, Article 86 § 1 of the Act. of August 29, 1997 of the Tax Ordinance, Article 74 paragraph 2 point 4 of the Act of 29.09.1994. On Accounting (Journal of Laws 2018 item 395).

Klauzula informacyjna: Zgłoszenie uczestnictwa w konkursie Czarny Koń Wideoklipu
Na podstawie art. 13 Rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/96/WE (Dz. Urz. UE L 2016, Nr 119) dalej „RODO” informuję że:
1. Administratorem Pani/Pana danych osobowych jest Katowice Miasto Ogrodów – Instytucja Kultury im. Krystyny Bochenek, plac Sejmu Śląskiego 2, 40-032 Katowice, zwany dalej Administratorem; Administrator prowadzi operacje przetwarzania Pani/Pana danych osobowych.
2. Kontakt z inspektorem ochrony danych jest możliwy pod adresem:
- Katowice Miasto Ogrodów – Instytucja Kultury im. Krystyny Bochenek, plac Sejmu Śląskiego 2, 40-032 Katowice;

3. Pani/Pana dane osobowe przetwarzane będą w celu wzięcia udziału w konkursie Czarny Koń Wideoklipu i nie będą udostępniane innym odbiorcom, chyba, że wynika to bezpośrednio z przepisów prawa.
4. Podstawą przetwarzania Pani/Pana danych osobowych jest wykonanie zadania realizowanego w interesie publicznym - art. 6 ust. 1 lit. e) RODO.
5. Zakres przetwarzanych danych: imię, nazwisko, adres e-mail, ilość osób/zgłoszenie.
6. Podanie przez Panią/Pana danych osobowych jest dobrowolne i niezbędne do realizacji celu wzięcia udziału w imprezie; w przypadku niepodania danych niemożliwa jest realizacja w/w celu.
7. Na podstawie obowiązujących przepisów posiada Pani/Pan prawo do:
- dostępu do swoich danych osobowych (art. 15 RODO),
- sprostowania swoich danych (art. 16 RODO),
- usunięcia (art. 17 RODO) lub ograniczenia przetwarzania danych osobowych (art. 18 RODO),
- przenoszenia danych (art. 20 RODO),
- wniesienia sprzeciwu wobec takiego przetwarzania (art. 21 RODO),
- wniesienia skargi do organu nadzorczego (art. 77 ust. 1 RODO),
8. Pani/Pana dane osobowe nie podlegają zautomatyzowanemu podejmowaniu decyzji, w tym profilowaniu
(w myśl art. 22 RODO).
9. Pani/Pana dane osobowe będą przechowywane do terminu zakończenia festiwalu. W przypadku laureatów konkursu - Pani/Pana dane osobowe będą przechowywane przez okres 6 lat zgodnie z art. 70 § 1, art. 86 § 1 ustawy z dnia 29 sierpnia 1997 r. Ordynacja podatkowa, art. 74 ust. 2 pkt 4 ustawy z dnia 29.09.1994 r. o rachunkowości (Dz.U. 2018 poz. 395).
