Every year since 1976, Junior Auxiliary of Hammond has inducted an outstanding Man and Woman into our Hall of Fame. Please use this form to nominate an outstanding individual (no organizations) for this distinction. The nominee must have accomplished an outstanding humanitarian and/or civic deed or deeds, for the benefit of children (age 0-17 years) in the Hammond area. Any person or organization may submit multiple nominations.

Entry and Selection Process:

   • Individuals or organizations may submit names with qualifications.
 
  • Each individual or organization may submit more than one nomination. Nominations of the same name are encouraged annually.

   • Judging will be based solely on the information submitted with an individual’s nomination(s); therefore, please provide specific details of the individual’s contributions as completely and accurately as possible. Entries of more than 1,000 words will not be considered.
