Thank you for considering agreeing to your child taking part in this research.  Helen Devere-Catt must explain the project to you before you agree to them taking part.  If you have any questions arising from the Information Sheet or explanation already given to you, please ask Helen Devere-Catt before you decide whether to consent to your child participating. You will be given a copy of this Consent Form to keep and refer to at any time. If you consent to your child participating, your child will be given information and asked if they would like to take part or not. A small number of children (6-12) will be invited to take part in an interview/discussion with Helen Devere-Catt after the intervention has finished. This will involve discussion of how your child found the process of taking part in the intervention. Further information will be sent out prior to this to the parents of those children.

I confirm that I understand that by ticking each box below I am consenting to this element of the study.  I understand that it will be assumed that if I do not mark a box that I DO NOT consent to that part of the study.  I understand that by not giving consent for any one element that my child may be deemed ineligible for the study.

I voluntarily agree to my child taking part in this study. *
I confirm that I have read and understood the Information Sheet for the above study.  I have had an opportunity to consider the information and what will be expected of my child.  I have also had the opportunity to ask questions which have been answered to my satisfaction *
I understand that I will be able to withdraw my child’s data up to 6 weeks after the research has been completed. *
I consent to my child participating in the study. I understand that personal information belonging to my child (name, age, gender, and questionnaire responses) will be used for the purposes explained to me.  I understand that according to data protection legislation, ‘public task’ will be the lawful basis for processing. *
I understand that confidentiality will be maintained as far as possible, unless during data collection Helen Devere-Catt hears or sees anything which makes her worried that someone might be in danger of harm, she may have to inform relevant agencies of this. I understand that my child’s data gathered in this study will be stored under a pseudonym and securely within UCL systems.  It will not be possible to identify them in any publications. Data will be retained until any publications are accepted, after which it will be destroyed. *
I understand that my child’s information may be subject to review by responsible individuals from UCL for monitoring and audit purposes. *
I understand that my child’s participation is voluntary and that I am free to withdraw them at any time without giving a reason. I understand that I consent to my child taking part, my child will also be given the choice of whether or not to participate. *
I understand the requirements of my child taking part. School staff will provide support for my child if there are any concerns or distress resulting from participation. If my child does become distressed, they will be reminded of the option to withdraw from the research. If I feel my child is distressed or would like to withdraw I can contact the school to tell them. *
I understand the direct benefits of my child participating and the indirect benefits for the mental wellbeing of all adolescents *
I understand that the data will not be made available to any commercial organisations but is solely the responsibility of Helen Devere-Catt. *
I understand that the data gathered will be used as part of a doctoral thesis and possibly in publications and conference presentations. *
I confirm that I understand why my child has been invited to take part as explained in the Information Sheet. *
I am aware of who I should contact if I wish to make a complaint *
