2020-2021 NCW Middle School Registration
$300 per wrestler - includes t-shirt*, shorts* and team singlet*, bus expenses, referees and tournament fees.  Each wrestler must purchase their own AAU and/or USA wrestling card. (Team gear was ordered for those wrestlers registered by 10/26/2020)

As a parent or guardian of above named wrestler, I approve of his/her joining in this activity of Nashville Catholic Youth Wrestling (NCYW). I agree not to hold the club, the board of directors, officers, staff, or volunteers responsible and/or liable and hereby release them from liability for losses of any personal property and for any injuries or accidents suffered by my child at NCYW athletics/sports programs and related events and activities, unless such injuries or accidents result directly from negligence or willful act of an official of NCYW acting within the scope of his/her capacity. I have read and agree to the “Codes of Conduct” listed at www.nashvillecatholicwrestling.com. I acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and/or death and severe social and economic losses which might result from the rules of play or the condition of the premises or of any equipment used. Further that there may be other risks not known to us or not reasonably foreseeable at this time. In consideration of the acceptance of this registration, I assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability, or death. To my knowledge, my child has no medical or physical condition that would keep him/her from or be affected by participation in this wrestling program. I provide permission and consent to the use of any photographs of my child in this program or on the NCYW website without compensation, for the advancement of NCYW.
