This survey is to determine your preferences as we begin to look at options for the Fall semester. As much as we would love to see every student in the classroom with their teacher, we know that COVID-19 guidelines still exist and we need to look at learning options for our students.

If we can not fully resume with in-person learning due to strict social-distancing guidelines, please select all options you would be willing to consider for your child to begin in the Fall? *

Hybrid Approach: In-school 2 days per week, distant learning 3 days per week. (Example: Your child attends in-person learning on Monday/Wednesday with distant learning on Tuesday/Thursday/Friday.

If the school can meet social-distancing guidelines, I would be willing to send my child to school everyday.

I would be willing to to enroll my child in the online Hematite Academy.
