This form allows you to let us know you would like raise a concern. It will be received by the Member Protection & Information Officer (MPIO) and prioritised accordingly based on the topic.

A concern can be raised by anyone (a child, young person, family member, umpire, official, senior player, coach, manager, club member or member of the community) and be about anything (fixtures, facilities, behaviour of another young athlete, behaviour or actions of an adult, situation risk, any other concerns relating to LTUHC). If you wish, you can Raise A Concern anonymously, however we will be unable to reply to you or provide you updates on the concern.
