COACHING TERMS & CONDITIONS:

Clients are allowed one make up session if they need to cancel less than 24 hours in advanced. However, coaching sessions that are not rescheduled or cancelled 24 hours in advance will result in forfeiture of the session and a loss of the financial investment at the rate of one session.

Clients arriving late will receive the remaining scheduled session time, unless other arrangements have been previously made with the trainer.

The expiration policy requires completion of all personal training sessions within (days to be determined by Project Health & Wellness, LLC) days from the date of the contract. Personal training sessions are void after this time period.

No personal training refunds will be issued for any reason, including but not limited to relocation, illness, and unused sessions.

As of 5/15/2019 Project Health & Wellness, LLC will only accept credit, debit and cash payments. The company will send out invoices on either agreed upon date for as long as the sessions last. Clients are to pay their invoices in full on the date that the Invoice is sent out. If client does not pay, coach is not required to continue training client until payment is made.

Project Health & Wellness, LLC has the right to terminate agreement early if client is unable to pay for their selected service in a timely manner.

MEDIA RELEASE:

By checking "I agree" I hereby consent to and authorize the use and reproduction of any and all photographs, video footage or printed copy of me for Project Health & Wellness, LLC promotional purposes, including website use, social media use and physical periodical purposes. I understand that I receive no reimbursement for allowing my photo to be taken or for the use of the photo, video or my personal story.
