Thank you for choosing to participate in the Town of Meenon Comprehensive Draft Plan Comment Form! Your feedback is crucial for helping Meenon and its respective communities to be able to successfully implement the plan. This is a community product!

Just some quick House Keeping items before you proceed further with the Comment Form...

1) You are invited to participate in this Google-based Comment Form. The goal of this Comment Form is to capture your opinions and perspectives.

2) This Comment form was co-developed by the Town of Meenon (the Project Client Lead) and Northwest Regional Planning Commission (the Consultant). Northwest Regional Planning Commission receives all feedback from all Comment Form participants. The Comment Form feedback will be incorporated as content within the finalized copy of the Town of Meenon Comprehensive Plan for the purpose of re-visiting the communities and the county's feedback to understand how to address these ideas, concerns, and considerations, thereby effectively implementing a plan in an on-going manner that is relevant to the communities and Douglas County.

3) There are no qualifications to participate in this Comment Form.

4) Your participation in this Comment Form is 100% voluntary, therefore no contributions in any form will be provided in return to any Comment Form participant as a result of completing the Comment Form. If you do agree to participate, you will be given the OPTION to provide your name and contact information along with your interest in engaging in future efforts to support the implementation of the plan, and a short series of questions with an opportunity to provide additional comments to the Comment Form. You can decide which items and / or questions to respond to as ALL items and questions are OPTIONAL. If after you have submitted the Comment Form, you had changed your mind regarding a response(s) you provided, you may submit a new Comment Form OR you can simply choose not to complete a new Comment Form. Please note that your feedback will be recorded and assumed as being finalized by each Comment Form participant once received.

5) The information that is provided will be kept completely confidential and will only be used for purposes that will support the development of this plan and implementation of the plan.

6) Any feedback that is captured in this Comment Form that is considered vulgar, slang, and / or inappropriate, in any form, will not be captured as feedback to be included in the finalized copy of the Town of Meenon Comprehensive Plan nor will it be shared electronically in any way. The only exception being is if the vulgar, slang and / or inappropriate feedback is deemed necessary to be submitted to law enforcement to be used as evidence for investigative purposes.

8) We encourage youth under 18 years of age to participate in this Comment Form as all perspectives are important to consider! We require that a parent or legal guardian to please be present throughout the duration of the participants' completion of this Comment Form. Since it is assumed that you are a parent or legal guardian who is monitoring a youth's participation in this Comment Form, this Informed Consent Discloser assumes that you allowed and consented for the youth's participation of this Comment Form and you, as the parent or legal guardian, will assume full responsibility to any and all information that is provided once it is submitted.
