1. The presentation format will be for one 30-minute discussion. The workshop committee reserves the right to allocate more or less time as appropriate.

2. All presenters must present in-person at Aurora Conference Center in Milwaukee, Wisconsin. There is no virtual option.

3. Please complete all fields (except unneeded Secondary Presenters); use NA if it does not apply.

4. Presenters with accepted proposals will receive a $100 honorarium, however, travel expense reimbursement will NOT be provided for any presenter submitting through the Request for Proposals.

5. Presenters may not use any conference presentation to market products or services.

6. Every presenter, lead and secondary, will be required to declare any potential financial conflict of interest, i.e. any association or role that could, as a direct result of this presentation, cause a financial benefit to the presenter or their family.
