Jackson Intermediate School (Grades 3-5) Parent/Guardian Rostering Survey for 2023-2024  
Each year we consider many factors when placing children in classes at Jackson Intermediate School. Because we strive to provide the most positive experience for all students, and because we value your input regarding your child/children, I ask that you take some time to complete this questionnaire and submit it by Friday, May 12, 2023.

Please complete/submit this form for your student(s) who will be entering 3rd, 4th or 5th grade at Jackson Intermediate School next school year (2023-24).  

Please note that I don't accept requests for specific teachers since staffing changes can occur unexpectedly during summer months with reassignments, retirements and resignations. Be assured, however, that your input, along with that of our teachers, will enable me to make the best possible class placements for all our children.
