Please complete this COVID-19 symptom checklist each day before 7:20 a.m.  Completing this screening before sending your child (or children) to school will allow us to proceed with drop-off quickly and efficiently.

If the answer to any of the questions indicates that your child is experiencing symptoms consistent with COVID-19, please keep your child at home and contact to the school office (314) 752-4700 to report their illness.  See our Illness Protocol Chart for what to do next.
