Sliding Scale/Low-Fee Care

Please note that I offer sliding scale/low-fee care to individuals with incomes at or below 300% of the Federal Poverty Guidelines for a household of 1 (as published yearly in the Federal Register at www.federalregister.gov). Eligibility for sliding scale care is reviewed periodically.

Current sliding scale openings:

- Waitlist: Psychotherapy alone or Psychotherapy + Medication Management

To Qualify For Sliding Scale/Low-Fee Care

Please complete the following questions and have on hand at least one of the following items of documentation:

- most recent 1040 tax form

- unemployment check stubs

- summary of benefits letter from a social service agency

Insurance and Reimbursement

Although I am out-of-network with insurance plans (including MediCal/Medicare), I can provide you with a superbill that you can submit to your insurance for reimbursement. This may range from 50-100% of fees. I partner with Reimbursify, a service that makes it easy to submit your reimbursement claims. Learn more about reimbursement eligibility here.

Payments and Fees (Non-Sliding Scale)

15 minute initial phone call: no cost

75 minute initial consultation: $900 45 minute session: $600

25 minute session: $400 Forms/paperwork/refills/extended phone calls outside of appointments: $100 (based on 10 minute intervals)

Short administrative phone calls under 5 minutes: no cost
