TAYS 2019-20 Preliminary PE Waiver Identifier (This waiver is only for students who will be attending TJH 2019-20 school year.)

This form is to be completed if a student will be submitting a PE waiver in the fall. Students can turn in PE Waivers August 1, 2019 - August 16, 2019. This will allow the counselors to have the schedules ready for schedule pick up before school. PE WAIVERS WILL STILL NEED TO BE SUBMITTED TO THE COUNSELING OFFICE FOR APPROVAL. The waiver application can be found on the TAYS JH Counseling website.

Category 2 (5-14 hours during the week) Students will be placed in a study hall in lieu of P.E.
