2023 Special Persons Day Form

Please fill out this electronic form by Thursday (2/1) for Special Person's Day. It is important that we know who your child will be released to (relative or family friend 18+ older).

Note that the school day begins at 7:50AM and Mass begins at 8:00 AM, which will include Blessing of the Throats for the feast of St. Blaise. Students and their Special Person may sit together at Mass near the area where 6th grade usually sits (middle right side).

Students may also be dismissed with their Special Person when school is over at 12 PM or they may be signed out FROM THE CLASSROOM after Mass. Once students are signed out they are considered the adult's responsibility.

NOTE: If your student will not have a Special Person coming on Friday 2/3, please write "NONE" in the questions asking for the Special Person's name and relation.
