By signing this form, you will waive certain legal rights. Please read carefully! This is a binding legal agreement. In the Mosinee Hockey Club’s Mosinee Youth Ladies 3x3 Classic the undersigned acknowledges and agrees to the following terms.

DISCLAIMER

Mosinee Hockey Club, its directors, officers, members, employees, volunteers, officials, participants, clubs, agents, sponsors, owners/operators of the facility, and representatives (the “Organization”) are not responsible for any injury, damage or loss of any kind suffered by a Participant during ice rental.

RELEASE OF LIABILITY

In consideration of the Mosinee Hockey Club’s Mosinee Youth Ladies 3x3 Classic, I agree:

1. To waive any and all claims that I/my child may have against the Mosinee Hockey Club and their directors, officers, employees, agents and representatives, and any volunteers in any way associated with the event/program/game/ice rental, (all of whom are hereinafter collectively referred to as "the releasees");

2. To release the releasees from any and all liability for any loss, damage, injury or expense that I/my child may suffer or that my next of kin may suffer as a result of my/my child’s participation in the event/program/game/ice rental due to any cause whatsoever, including any negligence, breach of contract, breach of statutory duty of care, or breach of the occupiers' liability act on the part of the releasees;

3. To hold harmless and indemnify the releasees from any and all liability for any property damage or personal injury to any third party, resulting from my/my child’s participation in the event/program/game/ice rental; and

4. That this release of liability shall be effective and binding upon my heirs, next of kin, executors, administrators and assigns in the event of my death.

ACKNOWLEDGEMENT

You expressly acknowledge that you have read this agreement and understand the rights, obligations, terms and conditions set forth herein. You acknowledge that you have read and understand this agreement and are aware that by signing this agreement you are waiving certain legal rights on behalf of yourself and/or your child. You have executed this agreement voluntarily, and that this agreement is to be binding upon yourself, your heirs, executors, administrators and representatives.
