Please note: Unless this form is completed indicating your student needs transportation for the 2022-23 school year, they will not be assigned a bus.

Please note: High School students and Middle School students transitioning to High School will ride the city bus should they need transportation. Do not complete this form if your student is in High School currently, or will be in High School for the 2022-23 school year.
