Declaration by the Parent :
a. I certify that I am the bonafide guardian of the child.
b. I understand that the registration & admission fee is non refundable.
c. I hereby certify that the date of birth & spelling of the name of my child given in this form are correct to the best of my knowledge & I shall not make any request for changes in either.
d. Till the Birth certificate and Transfer Certificate of the child is deposited, his/her admission will be deemed provisional.
e. I understand that rendering false or misleading information or withholding correct information may disqualify the child for admission.
f. Once admitted, the child will not be entitled to refund of fee deposited except caution money, irrespective of when the child was admitted, in the case of the parents wishing to withdraw him/her.
g. I have made satisfactory arrangements for remittance of school fee within due date without waiting for a reminder from the school.
h. I here by certify that in case I do not claim the caution money paid by me for a period of three years after my ward leaves the school, my right over the refund of this amount will stand relinquished by me.
i. I have also understood that bus facility is optional & school is not bound to provide transport. It will not be obligatory on the part of the school to create new bus stops. It will be the sole responsibility of the parents to escort the ward to and from the respective bus stop.
j. Having read carefully the rules, regulations & procedures laid down in the school prospectus and being desirous of having my child educated in S.V.M. I hereby agree to abide by them in all respect. I understand that the decision of the management of the school shall be final and binding on me.
k. I hereby certify that my ward and I shall follow all the rules, regulations & procedures laid down by the school from time to time.
