As part of our risk management policy, we require no less than two adults (one of which must be a certified coach) to be present at all EYSA events. All contact with minor athletes must be observable and interruptible. While the focus of this policy is on the protection of the child, it also provides a level of protection for the volunteer(s).

Please complete this form any time you will not have at least two certified coaches available to attend a practice or game and will require an acceptable substitute in place of one of the coaches. An acceptable substitute is someone who is at least 18 years old and does not have any familial relationship to the certified coach in attendance. At least one of the adults in attendance for the event needs to be one of the certified coaches registered to the team. The certified coach in attendance is responsible for completing this form.
