Form for new clients to apply for registration as an assistance dog in training team with Laura Mundy Assistance Dogs. 

Thank you for your interest in training with us towards achieving assistance dog status with your dog. The form below will provide us with a brief introduction to you, your dog and your goals for including your dog in the role of working with you as an assistance dog in the community. This information enables us to ensure we are assisting you and your dog in the best way possible. Through this information we are able to ascertain your needs, your current situation and supports as well learn all about your dog you’re hoping to have as your assistance dog (or your child's or other person you support) so that we can determine they will be a good match to your needs in an assistance dog. Please fill out the information below to the best of your knowledge. This application process will also include an in-person assessment of suitability for training as an assistance dog and handler team. Undertaking this assessment process does not in any way guarantee that you will be suitable to begin the training process following the assessment or commit you to continuing with us. If it is agreed that your dog is suitable to commence training as an assistance dog and you agree to undertake training with us, you will be required to sign our terms and conditions which will be provided at booking of our initial assessment appointment. 

Please also note that all of the information provided in this form is kept private and confidential. No information will be shared with any external parties without your permission and only will be shared with your permission if it is essential in providing you the required service and support. Any information gathered is only in order to provide you with the best and necessary service to achieve your goals of your dog becoming your assistance dog.

Once you commit to undertaking training with us you will be required to provide a copy of a letter from your treating clinician or medical professional describing the diagnoses relevant and the expressed need for an assistance dog to help improve your functioning and access to community as per legislative requirements. A medical clearance letter is also required to be attached from your treating veterinarian ensuring that your dog is physically healthy enough to work as an assistance dog for you. These forms must be provided prior to being able to be accepted into any training in public places. Once we receive your application you will also be forwarded our terms and conditions of service document, a link to our privacy and complaints policies (these can be found on our website) and a guidelines for handlers (also available via our website). The terms and conditions of service must be signed and returned prior to our in-person assessment. 

The cost of the assessment of suitability for trainee assistance dog status is $250 and is non-refundable should the assessment deem you're not a suitable team for assistance dog training by Laura Mundy Assistance Dogs. 

Please note that we do not accept applications for dogs aged over 7 years at the time of application and that  we recommend a maximum working age of 11 years of age. Exceptional circumstances may allow some flexibility in this criteria but it is not standard practice

Are you aware that training an assistance dog up to Public Access Test standard takes significant time and commitment. There will be no cases of doing Public Access Tests without training with us for a period of 6 months. Trainee teams must undertake a PAT within two years of training.  *
