FIRST-TIME ENROLLMENT INSTRUCTIONS:To enroll in the Executive Function Skills Group for Adolescents, please complete the following steps:
1. Complete this brief questionnaire.
2. Once completed, we will send you a link to create an account on our portal, Jane App, where you will find consent forms, a release form, and intake paperwork. **These look like duplicate e-mails but each one contains different information so please open all 3 e-mails from Jane App.**
3. Use your Jane App portal to pay for the workshop ($225 +tax) to secure your spot.
All payments are due before April 5, 2024.
