Transportation Request Form 2023-2024

This form is for students that are new to the District and students that did not ride the bus during the 2022-2023 school year.   (If you have had a change of address please make sure that your student's school has the correct address and the change has been made in PowerSchool and you will need to see the Transportation Webpage for a Link to the Change of Address Transportation Request Form.) --- Again if nothing has changed about your student from last year (ie... home address and school) and your student rode the bus last year - YOU DO NOT HAVE TO FILL OUT THIS FORM

ALSO IF YOUR STUDENT WILL BE A CAR RIDER FOR MOST OF THE 2023-2024 SCHOOL YEAR PLEASE DO NOT FILL OUT THIS FORM

To request school bus transportation for your student please complete one form per student.  Allow up to 48 business hours for each request to be processed.

All rules and regulations concerning student ridership qualifications can be found on the District's Transportation Webpage.  The Transportation Department will only accommodate one pick up and drop off address per student per A.M. or P.M.

Every new request received in the Transportation Office completely overwrites every previous request.  Please make sure both A.M. and P.M. requests are complete.
