Louisiana DI Pin Order Form
Louisiana 2020 State Pins are pictured below and orders may be placed by completing the Google form below.

Team managers, please place an order for all the pins that your team members want to order so that we can ship them in one order to you rather than sending them to individual team members. That will help us save on packing time and postage. The cost for postage is included in the cost of the pins.

Step 1: Place your pin order by completing the Google Form below.

Step 2: Submit Form

Step 3: Make a check out to: Louisiana Destination Imagination (or LADI). Money orders are also acceptable. No cash please.  Also, note on your check how many pin sets you are purchasing for your team. Pins are $6.00 per set of 2. It is recommended that you take a picture of or keep a copy of your check.

Step 4: Mail payment ($6.00 for each set of 2 pins) to:

Pins will be mailed to you ASAP after payment is received.
