Halloween Trunk or Treat Registration
Extreme Youth Sports is hosting a FREE Halloween Trunk or Treat on Saturday, October 31 from 1-4pm.  It is an After School & Summer Camp Member's event.  Members' may invite one friend. Everyone must be registered to attend.  If you are not decorating a trunk, please bring in one bag of individually wrapped candy to donate. In order to promote safety at the event, we will be requiring masks, social distancing and will be providing gloves for any parent serving candy out of their trunk.  
 
**IMPORTANT:  This will be held at the Extreme Youth Sports Summer Camp and After School location, 300 Saint Laurent, Suite 152, Longwood, FL.

I understand that myself or my child must be healthy in order to attend the event.  I agree not to sue Extreme Youth Sports Inc. and or Extreme Youth Sports of Lake Mary, Kidz Success Zone, Inc., its owners, Managers, Agents, Employees, or Contractors for any injury or loss that could occur during the time my child is at Extreme Youth Sports. I give permission to use photos and video of my child in camp for normal advertising and training purposes.
