This is the official "LOSE2WIN2020" Church of the Living God (C.W.F.F.) weight loss challenge form. Make sure to read the following contest rules very carefully and fill out the form located at the bottom of this notice completely.

The Church of the Living God National Brotherhood has partnered with the American Heart Association working together to encourage the development and maintenance of healthy lifestyles for people in our local churches and their communities. “Lose2Win2020” is a comprehensive approach to fostering healthy living through promoting awareness of cardiovascular risk factors among African-Americans, the importance of healthier eating, regular exercise and weight loss.

This year's, "Lose2Win2020" involves a fun weight loss competition among local temples.  The three temples whose participants lose the most weight "per-capita" will be awarded first, second and third prices accordingly.

RULES:
 
• Weigh is on Sunday, January 5 through February 2,  2020 (Those who weighed in at the 2019 national convention must weigh in again with the participating temple)
• All participating temples MUST USE THE RECOMMENDED SCALE SOLD BY WALMART FOR UNDER $20 (See photos on announcement below or on our social media) –IF NOT AVAILABLE IN YOUR AREA PLEASE LET US KNOW BEFORE DEADLINE!
• A minimum of 6 participants and a maximum of 10 participants per group. The “majority” of participants MUST be members of the participating temple. (you can have up to two groups)
• Participants must be at least 18 years of age.
• There is a $10.00 entry fee for each person on your temple’s team. Funds should be taken at the time of weigh-in and will be collected in March 2020
• Each participant must present a frontal and side profile picture at weigh in. (this photo will only be used to show the progress of the "winning" teams)
• Each participating temple must complete the official forms completing the online form no later than Monday, February 3,, 2020 by midnight CST. (forms may be mailed) or completed on line; see above deadline.
• Only an ordained pastor can sign to validate and verify each participant’s official entry.
• Final weigh-in will be Sunday, July 19, 2020 at the national convention Tulsa, OK
• Each participating temple must complete the official final weigh-in forms no later than Monday July 20, 2020 by midnight CST-  
• In order to claim your prize, at least six participants from each team must be present at the 2020 National Convention on Saturday, August 1, 2020 in Tulsa, Oklahoma.
• Winners will be announced Saturday, August 1, 2020 at the national convention in Tulsa, Oklahoma.
• Every participant will be recognized.
• First, Second and Third place winning temples will receive a television to be used in ministry. (TV’s will be sent to their nearest Walmart Store)
