*This form is to be completed by a parent or guardian who makes a request for his/her child to be placed in the Honors Program at Freedom Middle School (FMS), even though the child has not met the qualifying criteria for specific classes. Please complete and submit this form for your child's 2024-25 school by May 31, 2024.

Reconsideration Statement
Even though my child’s scores have not qualified him/her for the Honors Program in FSSD for certain  classes, I respectfully request consideration be given for his/her participation in the program in the  subject area checked below in this form.
