PLEASE NOTE: COMPLETION OF THIS FORM IS NOT AN ELIGIBILITY CARD.  ELIGIBILITY MUST BE VERIFIED BY  YOUR COUNTY AGENT (based on your responses to this form).

Questions concerning eligibility card requirements for your county/club must be directed to your county 4-H agent. All 4-H'ers must have a current and complete 4-H Online enrollment.

Every 4-Her must complete a separate form on every project horse he/she expects to participate in within the current 4-H year. If you have more than one horse, you will need to complete this form multiple times.

Eligibility cards should be in possession of 4-H members when at any show to verify 4-H eligibility.

Ponies must be officially measured each year unless a permanent USEF or NCHJA card can be provided.

Eligibility cards will be used as a 4-H Pony Measurement Card, Rookie Walk/Trot, Short Stirrup, Long Stirrup, and Little Britches card. Retain eligibility cards for the current year and future reference.

To be valid, the parent, volunteer leader, and Extension Agent must all sign the card (no pencil).
