Panelist Sign-up Form for Considering Community Birth Q&A Sessions
Hosted by Blossom Birth and Family, these weekly sessions are designed to offer general information for those considering birthing at home or in a birth center with a midwife. Please use this form to sign up to be a panelist for all the sessions that work for your schedule.

During the session we will highlight your practice and invite you to share more about your work. The second half of our program will include a live Q&A session where participants can interact with our panelists. We are very grateful for your willingness to share your time and knowledge!
